How do I get my community member library card or renew my membership?

3 Public school students under the age of 18 may obtain a temporary library card. If coming to the library as part of a class, please note teachers must provide completed Public School Application for Library Privileges forms for each student 1 week prior to the scheduled visit to Kim Brooks or Lorraine Wochna. The form must be filled out by the student and their parent/guardian, including a valid parent’s/guardian’s Ohio driver’s license number (or State of Ohio ID card number), address, and parent’s/guardian’s signature.
